Factores clínicos asociados al éxito reproductivo de la fecundación in vitro en mujeres con infertilidad: revisión sistemática de la literatura, 2019–2025

Resumen

La infertilidad es una enfermedad del sistema reproductivo que puede afectar a la mujer, al hombre o a ambos integrantes de la pareja y cuyo tratamiento puede requerir fecundación in vitro (FIV). El objetivo fue analizar los factores clínicos asociados al éxito reproductivo de la FIV en mujeres con infertilidad mediante una revisión sistemática de literatura publicada entre 2019 y 2025. Se siguió PRISMA 2020 y una estrategia PECO. La identificación partió de 189 registros exportados de Dimensions y se complementó con nueve estudios primarios recuperados mediante búsquedas dirigidas y verificación en PubMed/MEDLINE y fuentes editoriales; 14 estudios primarios cumplieron los criterios de inclusión. Los desenlaces se jerarquizaron como nacido vivo, embarazo evolutivo y embarazo clínico, manteniendo implantación, fecundación y desarrollo embrionario como desenlaces intermedios. La edad materna mostró la asociación más consistente con menor probabilidad acumulada de nacido vivo; un índice de masa corporal elevado también se asoció con peores resultados en cohortes poblacionales ajustadas. La hormona antimülleriana se relacionó con el pronóstico, pero su interpretación dependió de la edad y del contexto clínico. La adenomiosis mostró asociaciones negativas con nacido vivo, mientras que la evidencia para endometriosis fue heterogénea. El grosor endometrial se asoció con nacido vivo sin justificar un umbral universal. Antecedentes reproductivos, TSH y hepatitis B no mostraron asociaciones independientes consistentes. El pronóstico de FIV debe interpretarse de forma multifactorial, específica para cada desenlace y ajustada por confusores.
